How to complete this diary download

Simply download and print the diary page, fill the diary in over the coming week, then take it with you to your next appointment with your GP/skin specialist. You can either use the diary page as a prompt for things you would like to discuss with them, or give them the completed sheet to read.

If you wish to keep a more extensive record (for example, a whole month) just print out more pages as required.
